W-4 Form [pdf]

The W-4 tax exemption form is required of all students prior to working in the Federal Work-Study or Lesley Work programs. Each student must complete a new W-4 form for every academic year that they work.

I-9 Form [pdf]

The I-9 identification form will be required of all students prior to working in the Federal Work-Study or Lesley Work programs. The I-9 form must be certified by the Financial Aid Office. Please bring the form and your identification with you to the Financial Aid Office in order to complete the form. If a student has worked in either the Federal Work-Study or the Lesley Work programs during a previous year, an I-9 form should already be on file for the student and the student does not need to complete another I-9 form.

Confidentiality Agreement [doc]

You may want to use this form if you work in an office where student employees will have access to sensitive information. Asking student employees to sign a formal agreement can help remind them of the seriousness of maintaining the confidentiality of the information with which they come into contact.

Performance Review [doc]

This form can be a useful tool for giving feedback to student employees. If you do not have a formal process for letting student employees know how they are doing, you may want to consider using this form or one like it to help your employees grow professionally.
