Online forms related to courses and communities in myLesley.

Two new forms have been added below for your convenience. 

Course Access Forms - Used by faculty to request special course access.

• Course Assistant/Special Guest/Mentor Access - For Student Teaching Assistants, Guest Presenters and Mentors.
• Development Shell Access - For faculty access to existing Development (DEV) course shells.
• Incomplete Grade Access - For students who need to attend a course as part of a make-up for an incomplete grade.
• Non-Credit Access - For students who need to attend a course as part of a non-credit enrollment.
• Independent Study Access - For students to attend a course as part of an independent study requirement.
• Practicum Access - For students who need to attend an alternate practicum course.

Community Forms

• Community Planning Guide [doc] - Faculty and staff use this form to submit a request for a new myLesley community. The form should be completed online and emailed to ut@lesley.edu.
• Mentoring Community Access - Mentors use this form to request that a mentee be added to their course mentoring community.  Don't have a course mentoring community?  Request one by using the above Community Planning Guide.
