HEALTH INSURANCEMassachusetts State Law requires ALL full or three quarter time students at Lesley University to participate in the school sponsored Student Accident and Sickness Insurance Plan or have an alternate insurance plan with comparable coverage. (Please note: Student status as full or three quarter time for the semester will be determined as of the last date of the add/drop period) To comply, students must either enroll in a Lesley-sponsored student health insurance plan, or provide proof of enrollment in an alternate qualifying insurance plan by logging into LOIS and selecting the Health Insurance link found under the Financial menu. It is the student's responsibility to ensure that the alternate insurance is adequate. Students must submit a waiver/enrollment form by September 15 for the fall semester or January 15 for the spring semester once per academic year. Late waivers will not be accepted. Health Insurance WaiverStudents who are covered by a qualifying insurance plan may waive their enrollment in a Lesley-assigned plan. In order to do this, the student must log into LOIS and select the Health Insurance link found under the Financial menu. Important Fall Dates Students may waive or accept health insurance through their LOIS account for the fall semester from July 16 through September 15. After September 15, the online waiver will no longer be available. After the September 15 deadline students must petition to waive or add health insurance directly through Gallagher Koster Insurance at www.gallagherkoster.com. Petitions are available from September 16 through October 15. After October 15 students may no longer make changes to their health insurance enrollment. Useful InformationDates of Coverage: The Student Accident and Sickness Plan covers you during semester breaks, summer vacation and even if you're studying abroad. You'll be covered for the period for which you paid premium. If you pay the premium for the entire year this year, you will be covered from September 1 through August 31 of the following year. Dental Insurance: Lesley University offers no student dental plan. However, included in the Student Accident and Sickness Plan is a dental savings program. The Dental Savings Program is an exclusive plan for students enrolled in the Student Accident and Sickness Plan. The program was developed by Basix, LLC to provide students access to general and specialty dental care from a select network of local dentists. The network of providers have met strict credentialing and quality assurance requirements. The network of participating dental providers have agreed to accept negotiated prices for the services they provide. Students will be responsible for paying for services they receive at the time of the visit. Student will generally save 50% of charges for a wide range of dental services - from routine cleanings to root canals. Because this is not an insurance plan, there are no claim forms, annual maximums, benefit limitations and condition or other plan provisions. Students can log onto the website. www.basixstudent.com to locate participating dental providers and the fee schedule. If you would prefer a dental insurance plan, there is a Student Dental Plan offered by the Boston University School of Dental Medicine to Lesley University students. This plan includes low-cost, high-quality care for students' basic dental needs such as cleanings and fillings.
